Physical Forms & Online Health Form

 

Each camper’s physical must be completed within ONE YEAR of camp date. Physician’s signature is required. Parents must certify that their student-athletes are able to participate in lacrosse camp and sign necessary waivers of liability during registration.

Player safety is emphasized in each drill and we utilize only certified athletic trainers with NCAA training experience. We have AED equipment and first-aid on site and we have an EAP prepared. We will not practice in environments where lightning is present (within 8 miles of campus).
